This site is designed to operate without advertising trackers or analytics by default. Form processing is disabled until an approved endpoint is configured.
Website privacy baseline
In this static build, the website itself does not intentionally set advertising cookies, run third-party analytics, or transmit form data unless a form endpoint is configured by the site operator.
Information you submit
Once a contact or intake endpoint is connected, information submitted through forms may be transmitted to and processed by that configured service. The site operator is responsible for selecting a platform and workflow appropriate to the information collected and the legal obligations that apply to the operating organization.
Sensitive information
The public intake page is intentionally designed not to request detailed medical records or diagnostic information. Detailed readiness, safety, referral, or clinical screening should be handled through an appropriate secure workflow.
Retention and access
Before launch, the operating organization should publish its actual retention period, access controls, sharing practices, and procedure for privacy requests. This page is a launch-ready structure, not a substitute for organization-specific legal review.
Changes
The privacy notice should be updated whenever analytics, forms, payment tools, scheduling tools, or other third-party services are added.
